We are seeking an experienced Care Manager to join our team and provide high-quality client care services. As a key member of our team, you will be responsible for delivering excellent client outcomes, managing care packages, and ensuring compliance with industry standards.

About the Role:

  • Deliver high-quality client care services, prioritizing individual needs and preferences.
  • Manage and oversee home care packages, including monitoring budgets and service fees.
  • Collaborate with internal teams to facilitate client onboarding and lead-to-conversion processes.
  • Ensure compliance with Aged Care Quality Standards, Home Care Packages, and Commonwealth Home Support Program requirements.
  • Proactively engage in continuous improvement across practice and service delivery.

Your Skills and Qualifications:

  • Experience in aged care, with knowledge of Home Care Packages and Commonwealth Home Support Program.
  • Frontline management skills, with the ability to lead and motivate a team.
  • Health-related qualifications not essential but highly regarded.
  • Procura program experience desired.

What We Offer:

  • A competitive annual salary of $65,000 - $90,000 AUD (dependent on experience).
