Support Worker Opportunity

Are you passionate about making a positive impact in the lives of others? Do you have a heart for helping vulnerable individuals achieve their goals and maximize their independence?

We are seeking a compassionate and dedicated Support Worker to join our team. As a Support Worker, you will play a vital role in providing high-quality case management, social work, and outreach services to older persons with diverse special needs.

Our ideal candidate is someone who embodies a person-centered, values-based approach to service delivery, informed by a proven commitment to social justice concerns. They should possess knowledge and understanding of trauma-informed, strengths-based, and person-centered care.

The successful applicant will have excellent communication and relationship management skills, as well as the ability to maintain collaborative working relationships with a wide range of stakeholders.

Key Responsibilities:
  • Provide support and assistance with living skills, connections with the community, personal and social development, employment, education, and training.
  • Assist participants with ADL's, money, health and wellbeing, general life skills, and recreational activities as part of their care management plans.
  • Create client file notes and assist with administrative responsibilities to ensure that operations function effectively.
  • Empower participants to pursue significant goals and make a positive influence in the lives of people.
Requirements:
  • Relevant qualification in Disability/Aged Care/Individual Support/Social Work
  • Full Victorian Driver's License and own vehicle registered in your name
  • National Criminal Check
  • Working with Children Check
  • First Aid & CPR certification (or willingness to obtain)
  • Covid-19 Vaccination Certificate
  • Manual Handling (not mandatory but preferable)
What We Offer:
  • Competitive hourly rate: $28.00 - $42.78 per hour
  • Shift work flexibility
  • Penalty rates applicable
