Job Description

As a Care Professional, you will be responsible for helping individuals with intellectual and/or developmental disabilities achieve their long-term and short-term goals by developing and implementing personalized plans.

These plans aim to enhance the overall quality of life for our members, and we rely on your exceptional relational and organizational skills to make it happen.

Our team members come from diverse backgrounds, but they all share a passion for delivering high-quality services to our members. You will have the opportunity to develop your skills through comprehensive training and work in a hybrid environment that includes office-based meetings and home visits.

If you support members outside of New York City, you will need a driver's license, reliable transportation, and appropriate insurance.

Our membership is diverse, and so is our workforce. We welcome bilingual candidates who can support this diversity and may provide additional compensation for fluency in languages such as Spanish, Korean, Mandarin, Cantonese, Russian, Hindi, Bengali, Urdu, Yiddish, Hebrew, and ASL.

All full-time positions come with generous benefits, including health, vision, and dental insurance, paid time off, $3000 tuition reimbursement per calendar year, and up to $250 of professional development courses.

Responsibilities
  • Advocate for and with our members to ensure informed decision making, consent, and guardianship.
  • Schedule, lead, and collaborate with interdisciplinary teams to conduct meetings and assessments ensuring comprehensive, person-centered plans.
  • Implement, update, and monitor plans, facilitating individualized reviews and approval processes.
  • Ensure integration of all necessary and preferred supports and services.
  • Maintain ongoing contact with critical individuals in members' lives.
  • Ensure timely submission of documentation.
  • Assist members with benefits and problem resolution.
Qualifications
  • A Bachelor's degree with 2+ years of relevant experience.
  • A License as a Registered Nurse with 2+ years of relevant experience.
  • A Master's degree with 1+ year of relevant experience.
  • Strong communication, interpersonal, and organizational skills.
