Senior Nursing Professional Opportunity

We are seeking a highly skilled Senior Nursing Professional to lead our patient flow and care coordination efforts at a renowned Sydney hospital.

  • Coordinate, lead and participate in projects related to patient flow and care coordination from one service location to another.
  • Maintain effective patient wait time monitoring and estimated discharge date management through the update and maintenance of electronic patient journey boards as well as identifying key 'Waiting for What' (W4W) and patient exit blockers.
  • Lead multidisciplinary meetings in relation to patient flow and movement, ensuring timely patient access. Provide expert guidance and negotiation with referrers, escalating any bed block issues.
  • Work collaboratively within a dynamic team to drive quality care delivery to community clients of the Palliative Care Service.
  • Provide evidence-based nursing care to people of all ages and cultural backgrounds, including individuals, families and communities.
  • Take an extended leadership role in the coordination of nursing and healthcare services, providing expert nursing assessment and triage to facilitate optimal health outcomes. Offer actionable feedback to staff on how to enhance patient satisfaction and efficiency.
