Job Description:

The role involves conducting comprehensive assessments using the Integrated Assessment Tool (IAT) to evaluate and determine the needs of customers seeking home support services. You will then develop personalised support plans and assist in the connection of relevant service providers, to help enable customers to retain their independence.

Key responsibilities include collaborating with customers, carers, families, representatives and other service providers in the aged care planning process; undertaking and reviewing personalised support plans, effectively managing changing customer and carer needs with their consent; identifying customers' formal and/or informal respite support and arrangements; developing an Emergency Care Plan if the customer does not have one in place; referring for services, assessing complexity and the risk of vulnerability, and assisting with linking support, reablement and short-term case management.

As a member of the assessment team, you will participate in clinical supervision and undertake various administrative functions, including report writing and correspondence preparation.
