Job Opportunity

We are seeking a skilled and compassionate Clinical Aged Care Professional to conduct comprehensive assessments and develop personalised support plans for customers seeking home support services.

Key Responsibilities:

  • Collaborate with customers, carers, families, and other service providers in the aged care planning process.
  • Develop and review individualised support plans, effectively managing changing customer and carer needs.
  • Identify and coordinate formal and informal respite support arrangements.
  • Develop Emergency Care Plans as needed.

Requirements:

  • A tertiary qualification in a health-related field.
  • Current registration or eligibility for registration with a relevant professional association.
  • At least one year of experience delivering services in aged care settings.
