Homelessness Case Manager Role

This is a dynamic opportunity to make a positive impact in the lives of individuals and families experiencing homelessness.

The Homelessness Stream provides high-quality, person-centred services to those at risk, or experiencing homelessness. Our team supports adults and families, including accompanying children with accommodation, case management, outreach support, financial assistance, connection and referral to other specialist services.

This role plays a key part in delivering our Homelessness Model of Care, guiding our services nationwide and setting a standard of care for those we support, partners, and the wider community.

As an Outreach Case Manager, you will provide comprehensive, client-focused case management to diverse cohorts at risk of or experiencing homelessness. Your primary objective will be to support clients in achieving housing stability, social inclusion, and stronger connections with community supports and services.

  • Deliver effective outreach case management to various groups at risk of or experiencing homelessness.
  • Conduct thorough intake and assessments using a trauma-informed and Housing First approach, aligned with our No Wrong Door policy.
  • Collaborate to develop individualised case plans tailored to each participant's goals, background, and needs.

Please note: This role may require occasional travel to regional NSW, depending on service needs.
