As a seasoned Care Coordinator, you will play a pivotal role in shaping our community healthcare services. This position demands exceptional leadership skills to effectively manage a team of Support Workers and champion our services within the local community.

Key Responsibilities:

  • Conduct thorough client care assessments and create personalized plans.
  • Oversee and lead a team of Support Workers, fostering their growth and development.
  • Coordinate intake processing, clinical care planning, and reporting tasks.
  • Schedule appointments and in-home services efficiently, considering staff discipline, availability, and geographical requirements.
  • Deliver high-quality person-centred care aligned with clients' needs and services.
  • Provide in-home care services as required.
  • Assume after-hours and weekend on-call duties.
  • Recruit and train new staff members.
  • Maintain regular contact with participants to ensure their needs are met.
  • Perform general administrative tasks.

Requirements:

  • A dedicated and experienced Care Coordinator with a strong commitment to client care.
  • A genuine passion for supporting vulnerable individuals within the community.
  • Excellent communication skills (written and verbal) to engage with providers, participants, and their families.
  • Confidence to intervene promptly in emergency situations.
  • Outstanding organisation and time management skills.
  • Proficiency in Microsoft Office Suite.
  • An understanding of privacy and confidentiality standards and regulations.
