Key Responsibilities:

Client Care
  • Providing person-centred care and support to clients with various needs.
  • Developing and implementing individualised care plans.
Collaboration and Communication
  • Collaborating with families, carers, and multidisciplinary teams.
  • Ensuring holistic care delivery through effective communication.
Documentation and Compliance
  • Maintaining accurate and timely documentation.
  • Complying with organisational and regulatory standards.

Requirements:

  • Certificate III (or higher) in Aged Care, Home and Community Care, or Disability.
  • Qualified and experienced professional with a minimum of three years' experience working in community or residential aged care.
  • Demonstrated knowledge and/or recent experience in aged care, including dementia care and manual handling.
  • Current Australian driver's licence, and access to a registered vehicle with a minimum of third-party insurance.
  • Current Police Check and Working with Vulnerable People (WWVP) registration, or willingness to obtain prior to placement.
