About the Role:

We are seeking a highly skilled Behaviour Support Practitioner to join our team in Shepparton. As an Individualised Care Specialist, you will play a vital role in supporting clients with complex needs to achieve their goals and aspirations.

Key Responsibilities:

  • Support clients to identify their strengths and interests through person-centred planning and assessment
  • Develop and implement evidence-based support plans that promote positive behaviour and well-being
  • Foster strong relationships with clients, families, and carers to ensure seamless support and care
  • Collaborate with multidisciplinary teams to deliver high-quality services and achieve outstanding outcomes

What You'll Need:

  • Current suitability with the NDIS Commission as a Behaviour Support Practitioner or equivalent experience
  • Demonstrated ability to work positively in a dynamic team environment
  • Outstanding communication, influencing, and interpersonal skills
  • Confident computer skills and proficiency in software applications
