About the Role

We are seeking a skilled Behaviour Support Practitioner to join our team in Shepparton (or nearby) location. This is an exciting opportunity for a dedicated professional to support our clients with their goals and aspirations.

This role involves working closely with our participants, their families, and carers to identify their needs through assessment and person-centred planning. You will develop, deliver, and evaluate evidence-based support plans within a complex support space, ensuring individual plans reflect our clients' goals, passions, and strengths.

Key Responsibilities
  • Support participants in identifying their needs and developing meaningful goals and strategies
  • Develop and deliver evidence-based support plans that meet the needs of our participants
  • Work collaboratively with the multidisciplinary team to provide comprehensive support services
  • Ensure participant outcomes are achieved through regular evaluation and review of support plans
