Care Coordinator Lead

The Care Coordinator Lead plays a pivotal role in ensuring the provision of high-quality care to residents. This involves collaborating with the Clinical Care Coordinator and Registered Nurse to supervise and coordinate care across the facility, while guaranteeing that individual needs, preferences, and goals are met.

Key Responsibilities- Provide emotional support to all residents, fostering a holistic and person-centered approach to care.
- Assist in coordinating the care needs of residents, encompassing medication management, nutrition, hydration, and pressure care management.
- Supervise care on shift, focusing on designated wings or areas.
- Perform medicine-related tasks according to scope of qualification, state/territory legislation, regulation, and Calvary policies.
- Relevant qualifications: Diploma of Enrolled Nursing (with APRHA Registration to administer medication) OR Certificate IV in Ageing Support (including HLTHPS007 Administer & Monitor Medicines) OR Certificate III in Individual Support with willingness to undertake training to obtain required qualifications.
- Commitment to integrity, honesty, and trustworthiness.
- Effective listening and communication skills at all levels.
- Ability to understand and utilize information technology and programs, as well as learn new skills as required.
- Current National police check.
- Working knowledge of WH&S and Infection Control.
- Demonstrated commitment to joining a mission-based organization dedicated to caring for others.
The estimated annual salary for this position ranges from $80,000 to $110,000 AUD, depending on experience and qualifications. Additionally, benefits include NFP salary packaging, discounted health insurance, paid parental leave, training opportunities, and flexible working hours.
