The Care Coordinator – Transport Services position is a key role within Capital Region Community Services Ltd, playing a vital part in delivering high-quality services to older people, individuals with disabilities, and those requiring short-term medical care.
In this position, you will be responsible for assessing, planning, and coordinating services and supports to enable individuals to optimise their choices and receive the care and support they choose, allowing them to live at home safely and independently.
As a Care Coordinator – Transport Services, you will work closely with participants, their advocates, and other colleagues to develop and implement effective care plans, ensuring the best possible outcomes for our clients.
Key Responsibilities- Recruitment, onboarding, supervision, and performance development of Community Transport Drivers
- Person-centred assessment, planning, and coordination of services and supports for older people
- Building effective partnerships with participants, their advocates, and other stakeholders
- Documentation and reporting requirements relevant to the role
- Quality audits, improvement plans, and achievement against quality standards
- Safety and risk management
- First Aid and CPR Certification
- Certificate IV or Diploma qualification in Aged Care related field (or working towards or willing to obtain)
- Working with Vulnerable People registration and National Criminal History Clearance
- Current Driver's Licence and access to a registered, comprehensively insured motor vehicle for work-related purposes
