About the Care Coordinator – Transport Services position

The Care Coordinator - Transport Services plays a crucial role in undertaking assessments, planning and coordination of services and supports for older people, people living with a disability and those requiring short-term medical care.

This position requires a caring and respectful individual with a solid understanding of how their support and the support of their team enables individuals to live with independence and dignity.

Job Description:

  • $73,941 - $87,233 per annum (dependent on experience)
  • A part-time ongoing position based at Trevor Pearcy House, Bruce. This role will require working from other locations as needed.

Key Responsibilities:

  • Recruitment, onboarding, supervision and performance development of Community Transport Drivers
  • Undertaking person-centred assessments, planning and coordination of services and supports for older people to enable them to optimise their choices and receive care and support they choose to live at home safely and independently
  • Building effective and collaborative partnerships with participants and their advocates, colleagues and key stakeholders
  • Documentation and reporting requirements relevant to the role
  • Following work instructions, best practice guidelines and safe work guidelines
  • Undertaking quality audits, developing improvement plans and achieving against quality standards
  • Feedback and complaints management and reporting
  • Safety and risk management

Requirements:

  • Certificate IV or Diploma qualification in Aged Care related field (or working towards or willing to obtain)
  • First Aid and CPR Certification
  • CRCS employees must hold a satisfactory Working with Vulnerable People registration and have this with them at all times whilst employed by CRCS.
  • Current Driver's Licence and access to a registered, comprehensively insured motor vehicle for work-related purposes
