Clinical Care Specialist Role

We are seeking a skilled Clinical Care Specialist to join our multidisciplinary healthcare team. In this role, you will coordinate and support the delivery of home-based health care services to clients across Australia.

This is a 12 month full-time fixed-term contract opportunity that offers a hybrid work model with 3 days worked from our office and 2 days from home.

  • Coordinate and manage Rehab at Home programs for clients discharging from hospital to home-based care following orthopaedic surgery or deconditioning post hospital admission.
  • Support and communicate with clients remotely to achieve their personal and clinical rehabilitation goals within their home environment.
  • Implement, monitor, and modify in-home care plans to meet client needs.
  • Manage clinical case escalations and variations to expected clinical progress.
  • Manage relationships with multiple stakeholders including hospital referrers, surgeons/specialists, service providers, and funders.
About the Role

In this Clinical Care Specialist role, you will be responsible for:

  • Developing and implementing comprehensive in-home care plans to meet client needs.
  • Coordinating with multidisciplinary teams to ensure seamless care delivery.
  • Providing education and support to clients and their families to promote independence and self-management.
  • Maintaining accurate records and reporting as required.

Key Skills and Qualifications:

  • Occupational Therapy, Physiotherapy, or Registered Nurse degree with current AHPRA registration.
  • Experience working in a multidisciplinary collaborative team environment.
  • Excellent knowledge of hospital discharge planning principles.
  • Ability to work in a dynamic environment with tight timeframes and competing priorities.
