
Healthcare Coordinator

The primary objective of this position is to enhance the health and wellbeing of Aboriginal and Torres Strait Islander peoples through comprehensive primary healthcare services.
Key Responsibilities- Deliver high-quality healthcare, including assessment, examination, treatment, and case management.
- Contribute to the development of holistic healthcare models that incorporate continuity of care.
- Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care Practice
- AHPRA registration as an Aboriginal Health Practitioner
- Effective communication skills with Aboriginal peoples
- Understanding of unique issues impacting Aboriginal health
- Experience in multidisciplinary teamwork and conflict resolution
- Sound written, computer, and communication skills
- Organizational and time management skills with adaptability
- Valid Working with Children Check (WA)
- National Criminal History Check Certificate (Police Clearance - valid within 3 months)
- Current C Class Drivers Licence
- Experience in Community or Aboriginal Health
- Hold a current Immunisation certificate
- Phlebotomy competency and currency of practice
- Knowledge and expertise in the use of Communicare or similar clinical database system
- Ability to travel on Outreach up to 4 nights at a time, if required
