Aged Care Service Professional

Our dynamic team is seeking a dedicated individual to fill this critical role in assessing the needs of older Australians.

This opportunity will allow you to make a real difference in the lives of our clients and take on new challenges while working collaboratively with colleagues to promote a supportive team culture.

You will work closely with multidisciplinary teams to conduct accurate assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support services.

We are looking for someone who has excellent assessment skills, a passion for providing quality care, and a commitment to continuous improvement.

About the Role:
  • Tertiary qualification in a health-related discipline directly related to Health, Aged Care or a specialist area such as nursing, medicine, occupational therapy or social work.
  • Current registration with the Australian Health Practitioners Regulation Agency (AHPRA).
  • An Australian driver's license and access to a registered, insured vehicle.
  • A minimum of one year's experience delivering services in aged care settings and/or to people living with dementia.
Key Responsibilities:
  • Collaborating with clients, families and multidisciplinary teams as required.
  • Conducting accurate assessments using the IAT.
  • Reviewing support plans to meet client needs.
  • Maintaining currency of knowledge relating to aged care services.
  • Assisting with frameworks to evaluate and enhance assessment processes and outcomes.
