Job Title: Aged Care Services Specialist

Role Overview

We are seeking a dedicated individual to join our Aged Care Assessment Services team on the Central Coast.

This role involves conducting assessments to identify and meet the complex needs of clients, and referring them to appropriate aged care services under the Aged Care Act.

Key Responsibilities:
  • Collaborate with clients, families, and multidisciplinary teams as required.
  • Conduct accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and determining eligibility for home support services.
  • Review support plans to meet client's changing support needs.
  • Work effectively both autonomously and as a member of a broader team in delivering to key performance indicators within identified timeframes.
  • Maintain currency of knowledge relating to aged care services both funded and non-funded.
  • Assist with continuous improvement frameworks to regularly evaluate and enhance assessment processes and outcomes.
Requirements:
  • Tertiary qualification in a health-related discipline directly related to Health, Aged Care, or a related specialist area (e.g., Registered Nurse, Medical Officer, Occupational Therapist, Physiotherapist, or Social Worker).
  • Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA).
  • Australian Driver's license and access to a registered, insured vehicle.
  • Minimum of one year demonstrated experience in Australia or Overseas directly delivering services in Aged Care settings and/or to Aged persons, including those living with dementia.
