Job Title: Clinical Aged Care Assessor

The Aged Care Assessment Services team is expanding, and we have a 6 month Fixed Term position available for a Clinical Assessor. This role offers flexibility and autonomy, working based on the Central Coast.

  • This role involves conducting assessments to identify and meet the complex needs of clients and refer to appropriate aged care services under the Aged Care Act.

Key Responsibilities:

  • Collaborate with clients, families and multidisciplinary teams as required.
  • Work collaboratively with colleagues to promote a supportive and positive team culture.
  • Conduct accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and determining eligibility for home support services.
  • Review support plans to meet client's changing support needs.
  • Work effectively both autonomously and as a member of a broader team in delivering to key performance indicators within identified timeframes.
  • Maintain currency of knowledge relating to aged care services both funded and non-funded.
  • Assist with continuous improvement frameworks to regularly evaluate and enhance assessment processes and outcomes.
Required Skills and Qualifications:
  • Tertiary qualification – in a health-related discipline directly related to Health, Aged Care or related specialist area (e.g. Registered Nurse, Medical Officer, Occupational Therapist, Physiotherapist or Social Worker).
  • Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA).
  • An Australian Driver's license and access to a registered, insured vehicle.
  • Minimum of one year demonstrated experience in Australia or Overseas directly delivering services in Aged Care settings and/or to Aged persons including to people living with dementia.
