At the forefront of aged care services, we're seeking a highly skilled and compassionate Clinical Aged Care Assessor to join our team. This pivotal role will see you working closely with clients, families, and multidisciplinary teams to identify and meet the complex needs of older Australians.

Your Key Responsibilities
  • Clinical Assessment: Conduct thorough and accurate assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support services.
  • Collaboration: Work effectively with colleagues to promote a positive team culture and collaborate with clients, families, and other stakeholders as required.
  • Continuous Improvement: Contribute to continuous improvement frameworks to regularly evaluate and enhance assessment processes and outcomes.
  • Qualifications and Registration: Possess a tertiary qualification in a health-related discipline directly related to Health, Aged Care, or a related specialist area, and hold current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA).

Key Requirements: To be considered for this role, applicants must have at least one year of demonstrated experience in delivering services in aged care settings and/or to older adults, including those living with dementia.
