Job Description- Conduct assessments to identify and meet the complex needs of clients
- Collaborate with clients, families, and multidisciplinary teams as required
- Work collaboratively with colleagues to promote a supportive and positive team culture
This role involves conducting accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and determining eligibility for home support services.
Day to day this includes reviewing support plans to meet client's changing support needs, working effectively both autonomously and as a member of a broader team in delivering to key performance indicators within identified timeframes, maintaining currency of knowledge relating to aged care services, and assisting with continuous improvement frameworks to regularly evaluate and enhance assessment processes and outcomes.

Required Skills and Qualifications- Tertiary qualification – in a health-related discipline directly related to Health, Aged Care or related specialist area
- Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA)
- Australian Driver's license and access to a registered, insured vehicle
