About the Role

We are seeking a dedicated and compassionate Primary Care Nurse Practitioner to join our team at the Centre of Clinical Excellence for Veterans and First Responders Health. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality, patient-centered care to our cherished Veterans and first responders.

Key Responsibilities
  1. Primary Care Services: Provide comprehensive primary care services to Veterans, first responders, and their family members, including assessment, diagnosis, and treatment planning.
  2. Patient Assessment and Care: Conduct thorough health assessments, diagnose, and develop treatment plans for patients with diverse healthcare needs, including mental health and addiction concerns.
  3. Chronic Disease Management: Manage and support patients with chronic illnesses, focusing on evidence-based treatment and prevention.
  4. Treatment Planning and Coordination: Develop and implement treatment plans, prescribe medications, order diagnostic tests, and interpret results.
  5. Mental Health and Trauma Care: Provide empathetic and compassionate care to individuals dealing with mental health issues, addiction, and trauma, ensuring holistic and patient-centered support.
  6. Patient Education and Empowerment: Deliver health promotion and disease prevention education, empowering patients to actively manage their well-being.
  7. Collaboration and Referrals: Collaborate with a multi-disciplinary healthcare team to ensure comprehensive and coordinated care, facilitating referrals to specialists and community resources as needed.
  8. Community Outreach and Engagement: Actively engage in community outreach programs and initiatives that support our mission and promote overall health and well-being.
  9. Clinical Record Keeping: Maintain accurate and up-to-date patient records and documentation in accordance with regulatory standards.
Requirements
  1. Master's Degree in Nursing
  2. Minimum 3 years of experience as a Nurse Practitioner
  3. Current licensure and certification as a Nurse Practitioner in British Columbia, with authorization to prescribe medications
  4. Passion for collaborating with experts from various fields to offer multifaceted care
  5. Experience caring for individuals dealing with complex health needs, mental health conditions, trauma, and chronic pain
  6. Strong interpersonal and communication skills with a patient-centered approach
  7. Proficient in the use of healthcare technology and electronic health records
  8. Commitment to evidence-based practice and ongoing professional development
  9. Compassion, empathy, and a dedication to providing high-quality, culturally sensitive care
