Key Responsibilities- Lead Care Outcomes: Coordinate and deliver person-centred care, ensuring the best possible outcomes for our residents.
- Oversight and Support: Provide oversight of residents' care, identifying changes in preference, needs, or wellbeing, and escalating support as needed.
- Comprehensive Care Planning: Develop and implement comprehensive care plans, conducting thorough assessments of residents' needs and preferences.
- Medication Management: Safely administer medications following established protocols.
- Documentation and Communication: Maintain accurate and up-to-date documentation, including admission assessments, care plans, interventions, and progress notes.
- Collaboration and Partnership: Work in partnership with our carers to deliver our Continuity of Carer model.
- Qualifications: Bachelor of Nursing degree or recognised equivalent qualification.
- Registration: Current AHPRA registration (Nursing).
- Experience: Experience in complex nursing clinical services and/or aged care setting, hospitals or community care.
- Desirable Skills: Experience in geriatric medicine, palliative care, or rehabilitation.
