Patient Care Manager

We are seeking a highly skilled Care Coordinator to join our team. The successful candidate will work in a dynamic environment, coordinating care and services to support patients in their own home.
Key Responsibilities:- Conduct initial phone assessments, care planning, and goal setting for clients.
- Coordinate in-home services to support clients receiving care at home.
- Build and maintain effective relationships with various stakeholders, including healthcare professionals and specialists.
- Monitor patient progress and update care plans as appropriate.
The ideal candidate will be an experienced clinician with a customer-focused approach and a strong ability to deliver best practice outcomes. They will have 3-5 years' experience in acute hospital settings, recent experience in care coordination is highly desirable, and excellent clinical reasoning skills are essential.
