
Senior Patient Care Manager

We are seeking an experienced Senior Patient Care Manager to lead a high-performing team of patient care specialists in a fast-paced, dynamic environment. This role is crucial to ensuring patients receive timely, accurate, and compassionate care, especially during critical moments.
The ideal candidate will have strong leadership skills, with experience in managing emergency medical and mental health cases, ideally in unscheduled care or telehealth settings. A proven track record of coaching and performance management is essential, as well as the ability to manage live, high-pressure incidents with clarity and confidence.
- Main Responsibilities:
- Lead and coach a team of patient care specialists to consistently deliver high-quality support across safety, medical, and mental health pathways.
- Manage daily operations in a 24/7 environment, ensuring optimal coverage, workflow efficiency, and resource utilization.
- Operational Oversight & Performance Management: Monitor performance to ensure the team is performing at its best in a high-pressure environment.
- Escalation Point: Act as the escalation point for complex, high-risk cases, including live incidents, emergency medical events, and critical mental health concerns.
- Monitoring Key Performance Indicators: Monitor key performance indicators and operational data to identify areas for improvement and implement strategies to enhance team and individual outcomes.
- Structured Onboarding: Deliver structured onboarding, training, and ongoing professional development sessions to keep the team sharp, informed, and confident.
- Multidisciplinary Collaboration: Support collaboration with medical and mental health professionals to ensure seamless care delivery.
