Job Title: Patient Care Specialist

Job Description:

We are seeking a skilled Patient Care Specialist to join our team. As a Patient Care Specialist, you will be responsible for delivering high-quality patient care in various settings, including patient homes, clinics, and via telehealth.

You will execute on the measurement, collection, and recording of vital signs, phlebotomy, and other essential tasks. Additionally, you will support functions such as answering telephone calls, retrieving medical records from external sources, completing patient intakes, and appointment scheduling.

You will also ensure patient consent is in place, obtain consent if not already completed, and support Nurse Practitioners for visits, follow up tasks, including patient outreach, educational materials, DME etc.

Serves patients and performs patient visits in multiple care settings as defined by role including patient home, clinics, partner MD space, and via telehealth/virtual visits.

Collaborates with cross-functional partners to ensure the delivery of holistic patient care.

Required Skills and Qualifications:
  • 2+ years combined of related education, experience, or certification.
  • Graduate from a medical assistant program such as Commission on Accreditation of Allied Health Education Programs (CAAHEP) or Accrediting Bureau of Health Education Schools (ABHES) Medical Assistant Program, OR Graduate of a formal medical assistant program.
  • Current BLS or CPR Certification required.
  • Efficient and reliable transportation, including an active driver's license, allowing for the ability to travel across an assigned region to meet patient needs. Locations may include offices, clinics, and patient homes.
  • Provides in-person patient care which may include standing, sitting, walking, pushing, pulling, and lifting.
  • Internet Connectivity - Min Speeds: 3.8Mbps/3.0Mbps (up/down): Latency
