About The Opportunity:

This is a fantastic opportunity for an experienced Healthcare Professional to join our Cardiovascular, Neurosurgical and Renal team.

The role involves delivering patient centred care that coordinates individualised care from admission and throughout hospitalisation, incorporating discharge planning in the plan of care.

  • Provides detailed documentation of a systematic comprehensive and accurate assessment of patients.
  • Coordinates and implements the intervention(s) identified in the plan of care/ clinical path.
  • Evaluates and records progress towards expected outcomes and revises the plan of care as necessary.
  • Utilises innovative and informed approaches using contemporary techniques and technology.
About You:

Must be a qualified healthcare professional with relevant experience.

Minimum 2-3 years clinical experience preferable with Cardiac experience.

  • Demonstrates high level of interpersonal skills.
  • Works well within a team environment and promotes team cohesion.
