Job Overview

We are seeking a Clinical Care Specialist to join our team. This is an exciting opportunity to work with clients in their homes, providing high-quality clinical care and support.

This role plays a vital part in delivering person-centred care, working closely with clients and key stakeholders to achieve their goals and needs.

Key Responsibilities
  • Clinical Assessment and Planning
    • Assess clients' clinical needs and develop comprehensive care plans.
    • Review and update care plans as needed to ensure optimal client outcomes.
  • Clinical Care Delivery
    • Deliver evidence-based clinical care in clients' homes, prioritizing their safety and well-being.
  • Collaboration and Communication
    • Work effectively with the care management team to achieve best client outcomes.
    • Provide guidance and support to care staff to ensure high-quality care delivery.
Requirements
  • Nursing qualification and current registration with AHPRA as a Registered Nurse (Division 1, General).
  • Proven experience in client-focused service delivery in aged and community/home care settings.
  • Knowledge of the Aged and Home Care Reforms and Aged Care Quality Standards.
  • Excellent communication and collaboration skills, with a commitment to advocacy and person-centred care.
  • Able to work effectively in a multidisciplinary team and independently across multiple sites.
  • A current driver's licence and ability to travel between locations.
  • Strong administrative and computer skills, including digital health records and care coordination tools.
  • Commitment to ongoing professional development and a patient approach to care.
Desirable Qualities
  • Nursing experience in aged care and community care.
  • Able to identify and implement innovations in care delivery and service improvement.
