We are seeking a highly motivated Care Coordinator to join our team in Canberra. As a Case Manager, you will play a vital role in providing support to senior citizens and their families.

About the Role

This is an exciting opportunity to work with a not-for-profit organisation that values its staff and the contribution they make. You will be working within a small office-based team, promoting services to build client numbers and actively involved in service promotion.

Key Responsibilities

  • Provide case management services to senior citizens and their families;
  • Assess clients' needs and develop individualised plans;
  • Work collaboratively with a small team environment to achieve organisational goals;
  • Promote services to build client numbers;
  • Actively participate in service development and quality improvement initiatives;
  • Develop and maintain relationships with community stakeholders;
  • Contribute to a positive and supportive team culture.

Requirements

  • Demonstrated experience in client assessment and case management in a community setting, preferably in aged care;
  • Sound understanding of relevant legislation, including the Single Funder Model;
  • Ability to work effectively with a culturally diverse group;
  • Excellent communication skills, both written and verbal;
  • Proficient in the use of computer software: Word, Excel, Internet, email, and client databases;
  • Demonstrated commitment to the 'Purpose and Principles' of Annecto – the people network.

Salary Estimate

$65,000 - $85,000 per annum, depending on experience and qualifications.
