A highly skilled Care Coordinator is required to join the Offender Reintegration team at Justice and Community Safety - ACT.

As a Disability and Complex Care Coordinator, you will provide specialized clinical care coordination for detainees with complex needs. This includes assessing individual needs, implementing clinical interventions, engaging with stakeholders, and conducting functional assessments.

Under direction, you will deliver a range of multidimensional treatment options for detainees with varied and complex needs. You will also provide advice on the care of and service provision for detainees with disabilities and complex needs, including contributing to accommodation unit placements and regular care coordination.

Maintaining monitoring systems to evaluate the effectiveness of treatment options in accordance with leading practice guidelines is crucial. Additionally, participating in the ongoing review and evaluation of work practices, including quality assurance and reporting on performance indicators, and contributing to the development of policies and procedures is essential.

To succeed in this role, you must have excellent interpersonal, negotiation, and communication skills. You should be able to deal effectively with people from diverse cultures and backgrounds in a correctional environment.

The successful applicant will hold registration as a Registered Nurse or Occupational Therapist and have a minimum of three years' paid work experience in a relevant role. Postgraduate qualifications are highly desirable. A current driver's license and Working with Vulnerable People registration are also required.

This position offers a salary estimate of $90,000 - $110,000 per annum, based on location and experience. Eligibility for permanent or temporary employment within the ACT Public Service requires being an Australian citizen, a permanent resident, or holding a valid work visa.
