As a Care Coordinator, you will utilise your knowledge and background in health to empower participants to self-manage their health. You will work collaboratively with healthcare professionals to facilitate health behaviour change and care coordination in the delivery of health coaching.
This is a unique opportunity to work from home and be located anywhere in Australia. You will be required to have a suitable office area and reliable internet connection.
Our service operates between the hours of 08:00AM - 6:00PM (AEST) Monday to Friday.
About YouWe are looking for an advocate for preventative health who has the natural ability to build rapport with a wide range of clients and healthcare professionals. You will have experience working in care coordination and delivery of support services to people with complex and chronic care needs.
You will be a Registered Nurse or Allied health Professional with relevant registration. In addition, you will have the following experience:
- Care coordination and the delivery of seamless and effective support services to people with complex and chronic care needs.
- Experience in primary health care and care planning.
- Strong clinical knowledge and the ability to translate care plans into practice.
- Strong understanding of chronic disease management practices, including workflows across the care team.
- Desirable but not essential: Previous experience delivering care virtually and digitally, and an understanding of health behaviour change methodology and how this can be applied in practice.
