Job Overview:

The Aged Care Assessment Officer is responsible for conducting comprehensive assessments of clients to determine their eligibility for aged care services.

\

Key Responsibilities:

\
  • \
  • Undertaking client assessments, reviews and development of support plans to assist clients in achieving their goals and living independently in their own homes and community.\
  • Participating in clinical triage and case conferencing to ensure seamless service delivery.\
  • Identifying, monitoring and managing risks associated with client care and leading appropriate escalation.\\

Requirements:

\
  • \
  • Demonstrated experience working in aged care settings and/or with older adults, with a focus on delivering high-quality services that promote independence and well-being.\
  • Relevant tertiary qualifications in a health-related discipline, such as nursing, social work or allied health.\
  • Current registration with the relevant professional association and/or regulatory body.\
  • Enrolled Nurses with at least one year's experience in aged care needs assessment may also be considered.\\
