Clinical Aged Care Specialist

We are seeking a highly skilled Clinical Aged Care Specialist to deliver high-quality care services.

  • Main Responsibilities:
  • Conduct thorough assessments of clients' care needs, considering their physical, medical, social, and cultural dimensions.
  • Develop personalized support plans that promote client choice and involvement in decision-making.
  • Manage an assessment caseload while meeting individual and team performance indicators.
  • Collaborate with healthcare professionals and other stakeholders to ensure comprehensive care.
  • Stay up-to-date with relevant legislation, funding guidelines, and service standards.
