Full-Time Case Manager

This is a full-time position in Ballarat. The role involves delivering the Journey to Social Inclusion program, providing long-term, personalized support to people experiencing chronic homelessness.
About the RoleAs an Intensive Case Manager, you will work with clients to access stable housing and sustain long-term tenancies. You will also increase your take-home pay with salary packaging benefits.
Key Responsibilities- Deliver the Journey to Social Inclusion program.
- Provide long-term, personalized support to people experiencing chronic homelessness.
You must have a qualification relevant to community services, such as Social Work or Psychology. Diploma qualifications with outstanding experience may be considered.
- You must hold a valid driver's licence.
- You must have demonstrated experience in case management.
- You must have proven ability to restore relationships with people experiencing vulnerabilities.
- You must be able to work effectively in an outreach capacity.
- You must have sound knowledge of theoretical frameworks and strategies applicable to working with clients with complex needs.
