Job Description

This position will support community members to meet their goals through community partnerships, collaboration and using case management as a tool.

We are seeking a dedicated Case Worker for The Doorways Program. This role will involve assessing community members' specific needs of interrelated issues and needs, developing high quality case plans with community members to identify achievable goals, providing effective response to people experiencing longer term disadvantage and supporting community members to build and increase capacity and develop independent living skills towards self determination.

This is a permanent part-time position, (14 hours per week), based in Ballarat, VIC. Salary and conditions are in accordance with SCHADS Level 4.
