Care Coordinator Position

We are seeking a highly motivated Care Coordinator to coordinate, manage and administer the delivery of care services to our clients.
Key Responsibilities:- Manage a caseload of 30-40 clients.
- Serve as the main point of contact for our clients for matters such as rostering, budgets and purchases.
- Provide guidance and technical advice to care staff.
- Be a leader and role model within our organization.
- Offer nursing support to clients (for ENs & RN applicants only).
- Advocate for clients in various matters.
- Clients require support with daily living activities.
- Clients need assistance with accessing community resources.
The successful candidate will provide nursing support to clients, specifically those requiring EN or RN level care.
