About the Role:

- Position: Enrolled Nurse Care Coordinator
- Location: Bundall, Gold Coast, QLD (Onsite)
- Hours: Full-time, Monday to Friday, 8:30 AM – 4:30 PM
- Reporting to: Care Manager
- Travel: Covering Gold Coast North & South regions

Key Responsibilities:

- Conduct client assessments and develop personalised care plans.
- Coordinate high-quality community care services and monitor client progress.
- Provide clinical guidance and support to care staff.
- Maintain compliance, accurate documentation, and care coordination.
- Work collaboratively with clients, families, and healthcare professionals.

What We’re Looking For:

- AHPRA-registered Enrolled Nurse with a passion for community care.
- Experience in aged care or community nursing (preferred).
- Strong knowledge of My Aged Care and Home Care Packages.
- Excellent communication, problem-solving, and organisational skills.
- Ability to work both independently and as part of a team.
