Community Care Coordinator Job

We are seeking a skilled Community Care Coordinator to join our team.

The ideal candidate will have a passion for delivering high-quality, client-focused care in a community setting. They will possess exceptional communication skills and be able to work autonomously or within a team environment.

Key responsibilities include:

  • Care Plans and Assessments
  • Government-Compliant Care Management Plans
  • Holistic outcomes driven Care Planning

Requirements:

  • Current registration with AHPRA
  • Community nursing experience is a must
  • Satisfactory Australian National Police check
  • Working With Children's Check
  • Current first aid and CPR certificate

Preferred Skills:

  • Time management, organisational skills & ability to manage competing priorities
  • Relationship development skills to support diverse clients to achieve their goals and objectives
