Nurse Care Specialist

We are seeking an experienced Registered Nurse to join our Residential Care Team at Southern Cross Care in North Plympton.
About the RoleThis is a fixed term part-time position (30 October 2024 - 30 April 2025) and a permanent part-time night duty position. The successful candidate will be responsible for directing and delivering high-quality nursing care to our clients, working closely with the Care Manager and Residential Services Manager.
Key Responsibilities include:
- Liaising with the Care Manager and Residential Services Manager to direct and deliver high-quality nursing care
- Leading and supervising support provision for daily living in accordance with each resident's care/service plan
- Assessing residents' condition and functional ability, ensuring risks are identified, managed, and evaluated
- Ensuring optimal use of medications, monitoring for adversity, and administering in a timely and safe manner
- Providing leadership, direction, support, mentoring, and coaching to Personal Carers, Enrolled Nurses, Hotel Service staff, Lifestyle, Maintenance, and Administration staff on each shift
- Ensuring clear and succinct handover from shift to shift and relevant documentation in the 7-day handover sheet correlates with progress notes, alert notification form, care plan suggested change forms, and risk management plans
We are looking for individuals who will make a positive impact on the lives of our residents and clients. The successful candidate will possess:
- Bachelor of Nursing with current AHPRA registration as an Registered Nurse
- Current First Aid Certificate (HLTAID011) & Current CPR Certificate (HLTAID009)
- At least 1-2 years of experience working as an Registered Nurse in an Aged Care Environment
- Sound communication skills with a level of English literacy that enables you to speak in a manner that older people can understand, as well as the ability to read written instructions and write clearly and concisely
- Exceptional time management skills to manage own workload to meet deadlines and support other staff
- Outstanding problem-solving skills to balance supporting residents' desire for independence and self-determination against potential risks and duty of care
- High-level understanding of the requirements to support residents living with dementia, including those who display challenging behaviours
- Understanding of how to apply AN-ACC to ensure optimum levels of funding are achieved
- Excellent contemporary best practice clinical knowledge, including medicines used in aged care
- Excellent knowledge of the Aged Care Standards, particularly as they relate to accreditation
