Job DescriptionThe Supportive Care Navigator is part of the Leukaemia Foundation's Triage & Navigation team, handling inbound referrals and phone calls. By conducting comprehensive intake assessments, the SCN offers initial support and connects clients to relevant information and ongoing supportive care.
This is a full-time permanent position based out of either our Brisbane site offices based at Dutton Park, or our Melbourne (Carlton) or Adelaide (Lightsview) offices. Applicants must be available to work between the hours of 9am – 4pm Monday to Friday AEST (9.00am – 5.00pm AEDST) including if they live in a day light saving state.Leukaemia Foundation adopts a hybrid working model, therefore if you have a suitable home workstation you will have the flexibility to work from home two days a week.
In this role you will be responsible for:
Inbound Referral Management & Assessment:
- Receive and manage all inbound referrals,phone calls and online chat from clients and healthcare professionals relating to supportive care services.
- Document and track all referrals and communications in the appropriate systems.
- Conduct thorough risk assessments and supportive care needs assessments for clients.
- Identify and prioritise the needs of clients to determine the most relevant services.
Supportive Care Triage & Coordination:
- Provide clients with accurate information regarding blood cancer and supportive care services, including relevant external supports.
- Appropriately triage, coordinate, and facilitate access to supportive care services across the suite of Leukaemia Foundation's service offerings, including digital, 1:1 supports, and practical services, e.g., financial assistance.
- Initiate internal / external referrals where required.
- Conduct follow-up calls and evaluations to ensure the effectiveness of the provided services where required.
- Work collaboratively with healthcare professionals (internal & external)
- Maintain accurate and up-to-date healthcare records, ensuring compliance with organisational policies, regulatory requirements, and health service standards.
Patient Safety & Quality:
- Participate in team meetings and continuing professional development activities to stay current with best practice in supportive care and blood cancer.
- Contribute to quality improvement initiatives within the Health Services team.
- Ensure provision of safe, high-quality care that complies with regulatory requirements, organisational policies, and healthcare standards.
Suitable applicants will have:
- Tertiary qualification in social work, nursing, or allied health profession.
- Australian Health Practitioner Regulation Agency (AHPRA) registration or membership of respective professional body.
- Comprehensive understanding of blood cancer and its impact on clients.
- Knowledge of supportive care services and resources available to blood cancer clients.
- Strong interpersonal and communication skills, with the ability to provide empathetic and compassionate support over telephone and digital modalities.
- Excellent organisational and time management skills, with the ability to prioritise and manage multiple tasks.
- Proficiency in using IT systems and maintaining detailed and accurate patient records.
- Demonstrated ability to work effectively both independently and as part of a multidisciplinary team.
- High level of emotional resilience and ability to handle sensitive and distressing situations.
- Commitment to continuous professional development and improving client outcomes.
- Experience in haematology/oncology settings and or supportive care in cancer services.
- Training in counselling or psychosocial support.
