Key Responsibilities- Provide health promotion and education to clients and carers, promoting healthy lifestyles and disease prevention.
- Promote and deliver our award-winning Wellness for Independence programs, empowering clients to take control of their health and wellbeing.
- Monitor and promote health outcomes via Telehealth, ensuring timely and effective care for our clients.
- Provide direct care services across a range of funding programs, including wound care, diabetes care, medication management, stoma care, and catheter care.
- Assess, develop, and evaluate nursing care for all clients, working collaboratively with multidisciplinary teams to achieve optimal health outcomes.
- Work with clients to develop and achieve client-centred care plans, promoting autonomy and independence.
- Assist clients to become independent with managing health needs through a Wellness and Reablement approach, working in conjunction with Allied Health clinicians.
- Division One Registered Nurse with the Australian Health Practitioners' Regulation Authority (AHPRA).
- Experience working as an independent clinical practitioner or in a team, with a strong understanding of community nursing and primary health care principles.
- Proficient in the use of electronic technology, including smart phones, tablets, laptops, email, internet, and client data management systems.
- Current driver's licence and ability to travel for client care.
- NDIS Worker Screening Check or willingness to obtain.
