
Public Health Advocate

A Community Health Champion will collaborate with individuals, families, and community groups to foster healthy lifestyles and reduce health disparities.
The role entails working alongside patients, healthcare professionals, and local organization staff to identify needs, provide support, and connect people with resources addressing physical, mental, and social health challenges.
Responsibilities:- Work closely with a designated group of individuals and their households to deliver personalized support in addressing the root causes of poor health and wellbeing.
- Take a holistic approach to address physical, mental, emotional, and social health and wellbeing, as well as resilience.
- Utilize various methods to enhance patients' knowledge, skills, and confidence in tackling their own problems and health-related challenges.
- Partner with individuals as equals, actively listening to their concerns, building trust and rapport to help them recognize their needs, and connecting them with suitable support or interventions.
- Help individuals identify barriers preventing them from accessing local resources or services effectively.
The Community Health Champion will also be responsible for identifying health and wellbeing needs through home visits, community outreach, and data analysis.
Required Skills and Qualifications:- Knowledge of the local health and social care system and available community resources.
- Understanding of confidentiality, safeguarding, and data protection principles.
- Approachable and friendly demeanor.
- Demonstrates empathy and compassion.
- Understands and cares for individuals facing health challenges.
- Valid driving license and use of a vehicle for business purposes.
- Familiarity with the local area.
- Demonstrates innovative approaches to work.
- Good command of English and mathematics.
- Care Certificate.
- Previous experience in patient-facing roles dealing with individuals and families.
- Previous experience managing a caseload and delivering outreach services.
