
Public Health Advocate

We are seeking a highly skilled and dedicated Health Promotion Specialist to work with individuals, families, and community groups in promoting healthy lifestyles and reducing health inequalities.
The successful candidate will provide personalized support to address causes of poor health and wellbeing, taking a holistic whole-person approach to physical, mental, emotional, and social health and wellbeing. They will use various methods to develop patients' knowledge, skills, and confidence to tackle their own problems and challenges affecting their health and wellbeing.
Key Responsibilities
- Work with identified groups of individuals and their households to provide support addressing causes of poor health and wellbeing.
- Develop patients' knowledge, skills, and confidence to tackle their own health and wellbeing challenges.
- Use a holistic approach to physical, mental, emotional, and social health and wellbeing.
- Help people identify barriers preventing them from accessing local resources or existing services.
- Identify health and wellbeing needs through home visits, community outreach, and data analysis.
- Understand local and accessible services and resources available and signpost to services supporting peoples'/families' health and wellbeing needs.
- Build relationships with local organisations and groups.
Requirements
- Good standard of English and Maths.
- Care Certificate.
- Previous experience in patient-facing roles dealing with individuals and families.
- Experience managing caseloads and delivering outreach services.
- Ability to work autonomously within the scope of the role.
- Skilled in daily use of Microsoft Office software and patient/case record systems.
