Job Opportunity

We are seeking a dedicated and compassionate individual to support vulnerable and marginalized older persons through high-quality case management, social work, and outreach.

As a Support Worker/Carer, you will play a vital role in helping individuals recognize their own self-efficacy and make long-term changes by taking control of their own journey.

The successful candidate will have relevant qualifications in Disability/Aged Care/Individual Support/Social Work, hold a Full Victorian Driver's License with their own vehicle, and possess certification in areas such as Manual Handling and First Aid & CPR.

Key responsibilities will include providing support and assistance with living skills, connections with the community, personal and social development, employment, education, and training. You will also assist participants with ADL's, money, health and wellbeing, general life skills, and recreational activities.
