Job Summary:

We are seeking a dedicated clinical social worker to provide direct care to patients and families in a hospital setting. This is a full-time, permanent position that requires working as part of a multidisciplinary team.

The successful candidate will conduct psychosocial assessments, advocacy, referral, supportive counseling, discharge planning, and case coordination. They will also navigate service systems such as Family Violence, Child Protection, NDIS, and My Aged Care.

To be successful in this role, the candidate should possess excellent communication skills, a strong commitment to patient care, and a keen interest in developing their skillset.

Required Skills and Qualifications:

  • A degree in Social Work and eligibility for membership of the Australian Association of Social Workers (AASW)
  • Previous experience in a hospital setting
  • Ability to work in a multi-disciplinary environment and contribute to case conferences and discussions
  • Demonstrated skills and knowledge in health, comprehensive psychosocial assessment, appropriate intervention plans, including discharge planning, responding to risk, collaborative care planning, and counseling
