Position Overview

The Clinical Care Manager (or Clinical Care Partner under the upcoming Support at Home Program) plays a critical role in ensuring the delivery of high-quality, person-centred care to older Australians living at home under the Home Care Package program.

This position is responsible for clinical oversight, care planning, monitoring client outcomes, and working collaboratively with clients, their families, support workers, and other health professionals to enable clients to live safely and independently at home.

Key Responsibilities
  • Conduct comprehensive initial assessments and regular reviews using a clinical and person-centred approach.
  • Develop, implement, and regularly review individualised care plans in collaboration with clients and their representatives.
  • Monitor changes in clients' health status and coordinate appropriate clinical and allied health interventions.
  • Ensure clinical care is aligned with Aged Care Quality Standards and other relevant legislation or frameworks.
  • Respond to and manage clinical incidents, risks, and emergencies as required.

Support at Home Transition (if applicable)

  • Adapt care delivery and planning in line with Support at Home Program requirements.
  • Act as the client's Clinical Care Partner — supporting their navigation of services and coordination of multidisciplinary care.

Leadership & Collaboration

  • Provide clinical guidance and support to care staff and support workers.
  • Liaise with GPs, specialists, allied health professionals, and family members to coordinate holistic care.
  • Participate in care coordination meetings and internal clinical governance activities.

Compliance & Documentation

  • Maintain accurate and timely client records, risk assessments, and care documentation.
  • Ensure compliance with organisational policies, government regulations, and clinical best practice.
  • Contribute to continuous improvement initiatives and audits as required.
