Job Overview

We are seeking a qualified Aged Care professional to conduct thorough assessments of older individuals and determine their care needs, preferences, and eligibility for aged care services.

Your Key Responsibilities

  • Conduct comprehensive assessments of older adults to identify their physical, emotional, and social needs;
  • Develop personalized care plans that support the individual's unique needs and circumstances;
  • Ensure all assessments are completed in accordance with current legislation, policies, and quality standards;
  • Collaborate with other healthcare professionals, government agencies, and service providers to coordinate services and ensure holistic care;
  • Advocate for the rights of older adults and ensure their voices are heard throughout the assessment and care process;
  • Monitor and review care plans to ensure they remain relevant and effective in meeting the changing needs of clients;

Requirements

We are looking for candidates with experience or qualifications in Aged Care, Nursing, Social Work, or a related field. Essential skills and knowledge include:

  • Strong communication, empathy, and assessment skills;
  • Knowledge of aged care services, policies, and regulations;
  • A commitment to delivering person-centered care and advocating for older adults' rights;
  • Ability to work independently and as part of a multidisciplinary team;
  • Advanced IT skills with previous experience working across multiple platforms (Microsoft Office Suite);
  • Previous experience performing in a KPI environment to ensure good quality client outcomes;
