Dietitian Grade 2

This position combines the responsibilities of a Care Coordinator and a Dietitian, Grade 2. As a Care Coordinator, you will work with clients who are at risk of homelessness, aiming to provide generic care coordination that facilitates their self-management strategies to improve health outcomes and prevent avoidable hospital presentations. You will assess eligible clients, conduct comprehensive assessments, coordinate health and social care needs, and ensure appropriate linkages with hospital and community services.
As a Dietitian, you will provide targeted dietary assessment, intervention, lifestyle coaching, and clinical management of dietary-related conditions and health behaviors. You will work with clients who have complex comorbidities, psychosocial issues, and financial difficulties that impact their nutritional status.
This role is part of the Health Care for Homeless Program (HfH) and has strong links with the Health Independence Program (HIP). You will work with an interdisciplinary team to provide support and interventions in the community.
Your Responsibilities- Assess eligible clients and conduct comprehensive assessments
- Coordinate health and social care needs
- Ensure appropriate linkages with hospital and community services
- Provide targeted dietary assessment, intervention, lifestyle coaching, and clinical management of dietary-related conditions and health behaviors
- Work with clients who have complex comorbidities, psychosocial issues, and financial difficulties that impact their nutritional status
- Bachelor of Nutrition Science
- Minimum 5 years' experience in a relevant acute/community setting
- Eligible for registration under the Accredited Practicing Dietitians
- Experience with prioritizing patients and hospital demands
- Demonstrated sound knowledge of the application of care coordination practice frameworks and principles in the current health care environment
- Current criminal history check or willing to obtain
