This position combines two key components: Care Coordinator and Dietitian Grade 2. You will screen referrals, recruit clients, conduct comprehensive assessments, coordinate health and social care needs, and ensure linkages with hospital and community services.
As a Dietitian, you will provide targeted dietary assessment, intervention, lifestyle coaching, and clinical management of dietary-related conditions and health behaviors. You will work with clients who have complex comorbidities, psychosocial issues, and financial difficulties impacting their nutritional status.
This role will receive clinical supervision from a Grade 3 Complex Care Dietitian and has strong links with the Dietetics department.
Your Responsibilities- Assess eligible clients with complex healthcare needs
- Provide generic care coordination to facilitate self-management strategies
- Conduct comprehensive assessments and coordinate health and social care needs
- Work with clients who have complex comorbidities, psychosocial issues, and financial difficulties
- Provide targeted dietary assessment, intervention, lifestyle coaching, and clinical management of dietary-related conditions and health behaviors
- Bachelor of Nutrition Science
- Minimum 5 years' experience in a relevant acute/community setting
- Eligible for registration under the Accredited Practicing Dietitians
- Experience with prioritizing patients and hospital demands
- Demonstrated sound knowledge of care coordination practice frameworks and principles
