About the Role

This position combines the skills of a care coordinator and a dietitian to provide comprehensive support to clients with complex healthcare needs.

As a care coordinator, you will assess eligible clients and develop personalized care plans to improve their health outcomes and prevent hospitalizations.

As a dietitian, you will provide targeted dietary assessments, interventions, and lifestyle coaching to support clients' health and wellbeing.

This role requires strong communication and collaboration skills, as well as the ability to work with clients from diverse backgrounds and with complex needs.

The ideal candidate will have a strong understanding of care coordination principles and practices, as well as experience working in a healthcare setting.

Key Responsibilities

  • Assess clients' healthcare needs and develop personalized care plans
  • Provide targeted dietary assessments and interventions
  • Offer lifestyle coaching and support to improve clients' health and wellbeing
  • Collaborate with clients, families, and healthcare teams to achieve care goals
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed

Requirements

  • Bachelor of Nutrition Science or related field
  • Minimum 5 years' experience in a relevant healthcare setting
  • Eligible for registration as an Accredited Practicing Dietitian
  • Strong communication and collaboration skills
  • Ability to work with clients from diverse backgrounds and with complex needs
